"plugin comes with 99 unique death effect"

Description lists 9 of them.  Can you post the entire list?

Can we limit the rotation to a sub-set of them and/or a single one?

The list is 9 effect categories. There are a number of effects within that.

Will update the listing with a separate list.

Also to answer your question, yes you can simply disable any effects that you do not wish to use, even if you only have 1 enabled.

Simply set the following field under the specific effect to false.

"enabled": false,

What should I do if I want effects to be random for everything that dies? I want to take advantage of the 90 effects and just have players experience them all

Leave it default. By default it picks random effects, and all 3 entity types are enabled by default (NPC, Animal and Player).

Can we get a deatheffects.exempt permission for those who don’t want it? I’m getting a few complaints lol

I'm going to post in my server info plugin how to toggle it on and off. But not everyone like to read. This would be a good option to have. If I see someone bitch in the chat history I can just turn it off on them. Done. Problem solved. Main offender seems to be when encountering a "zombie horde".

Hello can a player use the command /detoggle to toggle the effects for just themselves?

I disabled all the effects I didnt want players to be able to trigger and now only want them to be able to enable/disable their own effects. Is this how I do it?

Yeah its set per client from that menu I believe.

Can it be refined so that the kill effect is triggered only when using a specific weapon or skin, instead of just granting it to players via permission?

Nah it is setup to be player specific, not skin or weapon specific.

I've seen this plugin's kill effect used in some PVP add-ons, where it's tied to specific weapons with certain skins.  
It should allow the configuration to define which weapons can be obtained, and other players can earn them by killing the weapon’s owner.  
As long as a player has the weapon, they should be able to trigger the effect.

This should be a standout feature of the plugin and worth redesigning.  
I haven’t seen any other plugins offering this kind of functionality. If this goal can be achieved, I think it’ll be even more valuable!
