Normally the ship should start in the sky. Are you using any other plugin to modify the behavior of the cargo ship?

Lower mulitpliers are not supported by default as they might cause unexpected behavior. If you want to use lower multipliers anyways, they can be enabled by a flag. For more information see the Flags section in the description- 1,037 comments

This plugin only supports vanilla npcs. You will have to configure the loot for BetterNpc npcs manually. I am currently working on a full re-write of Loottable that also includes an option for other plugins to add custom crates/npcs. In the future it might be possible to configure the loot for BetterNpc directly in Loottable if the developer decides to add support for that feature

I didn't test the plugin with sky bases, but I assume that bases in the cargo flight path will be destroyed when the ship collides with them -

You can add extra items to an item. Extra items will always spawn when their parent item spawns
