Can you provide some more context to how this happened? Some questions to start with to narrow down the root cause:
Do you get any console error output? (check rcon, oxide/logs, oxide/logs/smartwarnings etc..)
Did the player come online after being warned (e.g. offline warning)
Did the player have multiple warnings unacknowledged
Did it keep the player frozen if you reload the plugin (e.g. freeze caused by something else)
Are you able to reproduce the scenario? (e.g. does it happen every time after a player has warned, or in any reproducible pattern?)
Please also paste a copy of your config file (remember to remove any sensitive information such as API keys)
