I don't understand if I can add or remove items from the vanilla profile.

that is, can I create a custom loot profile by working on the vanilla profile?

Yes you can edit vanilla profiles and add / remove items from them

i saw, i bought the plugin last month and now i'm trying it and it looks great: but so i can also use it instead of gather manager and stacksizecontroller?

yes, it is recommended to remove them so they don't interfer with each other
