More info would be nice. If it happens randomly when you load plugin on already running server - shit happens.

What information do you need?
Just loading the plugin is causing this problem a lot.

but it happens only once you load plugin and server is already running? if your server restarts with all the plugins everything is ok, right?

I have erased all configuration files, etc. and reinstalled this plugin but the error persists.
It is happening even when there is no player present.

i think you need to check your plugins then. how can OnPlayerAttack happen if there is no player present?

I do not know.
I am using better npc so is that the cause or something?
The spam to the console is not stable, sometimes it stops, sometimes it comes back.

When I checked today, the plugin was loaded but not working.
Also, when I reloaded it, it did not work correctly and I could only invite teams.

i'm aware of that problem if you are team leader, you can for now aim bit lower. But what you mean by "was loaded but not working" ?
