Loot issue with nightmare

Only the 4 base loot items are spawning in my nightmare bases. I have checked the loot tables and also re-installed the factory tier 3 tables and all the levels work except the nightmare. Any ideas?

this will only happen when there's an error. check the server console to see what the issue is. you want it to say that it loaded the Nightmare loot table, too, in addition to the Base Loot table. it has some 200 items in it
