Elevator spawn

I've loaded this on a map to test for wipe tomorrow.  I've wiped the test server twice to test it, and I haven't been able to get the elevator to spawn in.
Is there something I need to do in order to get this to work?  I've not seen anything listed as a dependency or configuration need.
