Hot Zones on Map

I'm having a bit of trouble with the hot zones appearing and disappearing on the map. It'll show a hot zone on the map and I'll arrive to that location, but it's not actually a hot zone, and vice versa. I'll come across a hot zone that isn't actually marked on the map. Is there something I'm overlooking?
