recycler not outputting

I'm having reports of the recycler not outputting. It takes input and recycles, but will not output. Am I missing something?

I just set up my own, it's broken, Items flow in to the recycler, but do not flow out or stay in the recycler. They just VANSIH in to thin air. GONE forever.
