Was working a few days ago but has quit working.  Any ideas?

I'm not aware of any issues. Are you admin / auth level 2?
if you do oxide.reload PermissionsManager in server console, does it successfully reload?

I made sure I had auth 2, did the reload and even downloaded a new one and still nothing.  I really like this mod, you have any other suggestions?

Did console confirm the plugin reloaded successfully, without any errors?

The main command is an in-game chat command - /perms - is that what you're doing?
Are there any console errors when you do that?

I totally uninstalled mods and put them back in.  TaDa it works. thanks for your assistance
