Hey! When exactly does this happen, does it happen all the time or just once?

Just now  so far just the one time   i noticed it when I was updating the ulimatelocker update..  Seems like when it first starts 
This was right after the error

I went in and force started the event it started   said it crashed  but no crash site on map
