Image is not showing up on button but shows in configuring GUI

HI David

I am not sure what I am doing wrong but I cannot get the image to show up on the button.  I can see the image in the GUI configurator but it never applies to the button.  I tried reloading and everything but all it has the is button location with the name on it which is clickable and all but not the image.  I appreciate your help.
