Can be from a few things. First reload the plugin and check your console to see if it gives an error at any point or isn't showing the files loading.

If everything loads properly generally this is conflict with another plugin calling the same hook. Sometimes it is harmless and doesn't actually result in any issues, sometimes its actually not going to work with whatever plugin is using that hook without some intervention.
